Hi mike- we have a house in CVO and visit regularly. Last month My daughter came with her 2 children and both got ear infections. They took them to the healthcentre in Lagoa - its signposted - and were seen by a doc and got the medicine (2 x 2 antibiotics), for a total of €18 - it took them approx 45 mins for the lot without an appointment.
Always remember to take your blue E111 card so you pay the same for the drugs as you do the locals.
On the adult side - I ended up with 2 broken toes last week at 9 at night - from calling the ambulance to back home after surgery in Portimao hospital to sew them back on (it was bad one!) we were back home by 1.30 pm including the 45 mins to find the 24 hr pharmacy in Portimao! Cost me €10 for the emergency treatment and €11 for the drugs
I have no complaints or worries about getting treatment in the local healthcare system.
Oh, and you are right the private centres are expensive. Dentists charge ~ €50 for a simple visit.

Doctors in Monte Carvoeiro are very good but also charge 50eurs first visit, but will come out to your property for emergencies.
