English Doctor or Nurse needed urgently

Does anyone know of an English Doctor or nurse who is here on Holiday, as I am in urgent need of a MATB1 form. I am about to have a baby here, but I need the form for Maternity leave in my job in the UK.

Thanks but the baby is due next week. I have been trying to sort this out for ages, so I know its a bit short notice.

This may still be useful if my company are ok, so thanks for the offer.

Hiya,
I do not know her name, but speak to Sara in 'Jokers' as she has a regular who is a British Nurse that comes in. I am sure Sara will be able to help you
