Ruff translation of requirements for touristic Liceance

Name………., resident……… fiscal nº……….., owner of a unifamiliar housing located in …….., Freguesia of ……., Council of Lagoa, enrolled at the respective matrix under the article nº……. and described at the Conservatória do Registo Predial of Lagoa under the nº ….., built through the Construction Licence nº…., of …./…./…., duly inspected by that City Hall and titleholder of the Licence of use nº …., of …./…./…., intending to affect it as a tourist housing, foreseen at art. 4º of the Decree nº 34/1997 of 17/09, comes to require your excellence the alteration of the use that was fixed before, and, for the effect, the realization of the competent inspection, with the purpose of being emitted the licence of touristic use, in the terms of the Artº 31º do Decree nº 55/2002 of 11/03 and in the terms of the al. e), nº2, of the Artº 4º do Decree nº 177/2001, of 04/06 (licence of use)/ al. f), nº3, of the Artº 4º do Decree nº 177/2001, of 04/06 (authorization of use), (adapt to the concrete case), in conjunction with nº3 of Artº 62º of the same decree, in the measure that doesn’t involve any construction that has to have a municipal licence.

For the effect attaches the following elements, in triplicate:

- Copy of the Original house plans approved by the City Hall & Habitation Licence
- Localization plants at the scale of 1/2000 and 1/25000;
- To date certificate emitted by the Conservatória do Registo Predial of Lagoa, that proves the legitimacy of the petitioner;
- Measurement sheet duly filled;

Attached the competent Project of safety against fire risks (in quintuplicate x 5), at the term of the Portarias nº 1063 and 1064/97, of 21/10.
